Do Italy citizens have to pay the single-entry visa fee?

On extensions or renewals it's not charged. The Foreign Ministry sets per-nationality reciprocity exemptions that aren't published on the goc.gov.tr page - if your case might fall under one, Mikato will confirm before you apply.
Can Italy students or work-permit holders skip the residence fee?
Italy citizens themselves aren't on the harç-exempt nationality list. But student and work-permit holders of any nationality - including Italy - are exempt from the residence harç under Law 492 Article 88: the $25/$5 harç drops to zero. The 964 TL card fee and (where applicable) the single-entry visa fee still apply.
